 You can request your military records in one of two ways, either online or with form SF - 180. Form SF - 180 can be downloaded or printed.

The SF - 180 form can then be mailed or faxed to the National Personnel Record Center (NPRC). The first page of this website will give you all the necessary instructions to obtain your military records including where to mail or fax the SF - 180 form.
